Here you will find the various forms that are required for children and youth to participate in our mid-week programs, as well as the Stay and Play Preschool Application.

​All forms are in pdf format, and may be downloaded and printed - or try our interactive online form, where applicable.  Please complete the required forms - with all required signatures -  and turn them in along with any applicable fees.

Children and Youth ~ LOGOS 

(Geckos and Chrysalis Grades 1-8) - Forms
Please complete the Registration and Health Release Forms - and return (or submit the online form) along with the program fee.
We are now accepting online payment for LOGOS program fees, click here to access the online payment page and select the "PROGRAM FEES ONLY: LOGOS and FISH" option.  Please click the box to include the 2% convenience fee if paying online.

    In the Sanctuary at  9:30 am, followed by our
    Exploration Hour (Coffee Available in the Social
    Hall from 9:00 - 11:00 am)

Summer Sunday Worship 
   In the Sanctuary at 9:30 am followed by Coffee
   Hour (outdoors as weather permits)

Live-Streamed Worship ~ An email is sent out on Saturdays  (to those on our mailing list) with a link to  our live-stream of the worship service on YouTube at 9:30 am on  Sundays.  If you would like to join our mailing list, please click here
