FISH Forms and Parent Information

• It's never too late!  Youth are welcome to join FISH at any time.  We meet on Wednesday evenings during the school year from 6:00 - 8:30 pm.
• If you wish to register for FISH, please download, print and complete the registration, insurance, and parent participation forms (see links below), complete forms online, or contact Martha Trout (814-238-2422) to receive them by mail.

Below are the Registration and Insurance Forms you can print, complete and return (along with the tuition payment) to register for FISH.  All forms for youth are required.

Parents: we encourage you to participate because we can't do it without parent volunteers -- and you'll have a great time!

Safeguarding Our Children and Youth Policy Requirements
View our Policies For Safeguarding Our Children (pdf).  You can get all the information about our policy which contains the PA state law requirements for EVERYONE who volunteers with our church youth in certain capacities.  Those who have regular contact with youth (or occasional drivers) are required to complete and turn in a volunteer registration form and get child abuse clearances (and FBI fingerprinting/Affidavit requirement)  Please see the policy for all the details.  If you are on a cooking crew, please complete a volunteer form.  If you are on a cooking crew or provide cookies for trips/retreats, you do not have to have clearances.

Please Note:  If youth is registering for either the Retreats/Mission Trip/End of Year Trip, you MUST be registered for the program by completing all REQUIRED forms and fees

We are accepting online payment for FISH program fees, click here to to access the online payment page and select the "Program Fees ONLY: LOGOS and FISH" option.  If paying online please make your payment in the amount designated as the online fee amount - this already includes the 2% convenience fee.
